Service Description Residential Habilitation aims to provide a supportive living environment where clients receive structured assistance to develop meaningful life skills and participate actively within the community. The service entails: • Structured Interventions: Programs that incorporate daily living skills training, social interactions, and community involvement. • Case Management: Continuous monitoring and development of individualized care plans, facilitating behavioral, educational, and health-related outcomes. • 24/7 Supervision and Care: Full-time support provided by trained direct support professionals, nurses, and care coordinators who are skilled in managing various developmental and intellectual disabilities.
