Step 1: Preparation and Scheduling 1. Initial Referral and Intake:

– Upon receipt of an inquiry, the intake staff completes a preliminary intake form, recording basic demographic data and preliminary service needs. – A pre- PAE checklist is utilized to determine whether the applicant’s history and available records indicate a potential need for ICF/IID level care.

Scheduling the Evaluation:

– The assessment team is notified of the referral, and an appointment is scheduled. – An introductory packet is provided to the applicant or their legal representative outlining the evaluation process, documentation requirements, consent forms, and privacy policies.

Consent and Documentation Collection:

– Written informed consent is obtained from the applicant or legal guardian. – Prior to the evaluation, the applicant is required to submit any historical clinical records, previous evaluations, and relevant medical documentation. – A dedicated liaison in the assessment team confirms receipt of these documents and verifies that they meet submission standards.

Step 2: Conducting the Comprehensive Evaluation 1. Administration of Standardized Assessments:

– The assessment team administers standardized tools such as intelligence scales (e.g., Wechsler scales or equivalent), functional adaptive behavior assessments, and other clinical measures. – Testing sessions are conducted in a controlled environment, ensuring consistency and adherence to established protocols. 2. Clinical Interview and Observations: – A structured clinical interview is conducted to gather detailed personal, developmental, and functional history. – The evaluator observes the applicant’s behavior, adaptive functioning, and communication skills. Detailed notes are recorded, covering both strengths and areas of need. 3. Compilation of Assessment Data: – All test scores, clinical observations, and interpretations are consolidated into a draft report. – The draft report includes a clear statement of the applicant’s level of care requirement, based on objective data and admission criteria under the CAC Waiver.
