Internal Reporting

All incidents must be reported first through internal channels. The process typically includes:

Initial Notification:

– The employee who observes or experiences an incident must immediately notify their direct supervisor through a phone call or via the established emergency reporting system. • Filling Out the Incident Report Form: – A standardized digital Incident Report Form (IRF) is available on the agency’s internal portal. Relevant details include the date, time, location, description of the incident, parties involved, and any immediate corrective measures taken. • Documentation and Review: – Once completed, the IRF is submitted electronically. Supervisors review the report during the next shift meeting, ensuring that all details are captured accurately. • Quality Assurance and Follow-Up: – The Quality Assurance Team monitors every incident report to ensure proper resolution. In cases where follow-up is required, a corrective action plan is developed and implemented promptly.

External Reporting

Certain incidents must be reported to regulatory bodies outside of Simpl Care Services LLC:

Regulatory Notifications:
