2. Neglect
Definition: Failure to provide necessary care and assistance, including basic needs, resulting in harm or risk to a client’s well -being.
Examples:
• Failure to provide adequate food, clothing, shelter, or hygiene. • Inadequate supervision leading to unsafe conditions or injury. • Not providing assistance with daily living activities when needed. • Ignoring signs of medical distress or failing to seek timely professional help.
3. Exploitation
Definition: The improper use of an individual for financial gain or other benefits by another party, including abuse of trust or manipulation.
Examples:
• Unauthorized use or theft of a client’s personal belongings or funds. • Coercing a client to relinquish decision-making power or personal resources. • Fraudulent practices that result in financial loss for the client.
4. Injuries and Accidents
Definition: Any incident in which a client or staff member sustains a physical injury or where damage to property occurs.
Examples:
• Falls within the facility causing fractures or head injuries. • Accidents during transportation or in-home care resulting in minor or serious injuries. • Staff injuries occurring during service delivery (e.g., lifting clients or handling equipment).
5. Medication Errors
Definition: Any deviation from prescribed medication orders, including wrong dose, wrong medication, incorrect route, or errors in timing.
Examples:
• Administering an incorrect dose of a medication. • Providing a medication that the client is allergic to. • Delays in administration leading to adverse health effects. • Failure to reconcile medications properly at points of care transition.
