Completing the Incident Report: – Use the standardized Injury Incident Report Form, which is available on the intranet and in the supervisor’s office. – The form must include details such as: • Date and time of the incident. • Detailed description of the incident. • Nature and extent of injuries sustained. • Names of any witnesses. • Immediate steps taken. – The report is to be filled out by the injured employee (if possible) or by a designated witness. 3. Investigation and Documentation: – The supervisor and OHS representative review the report and initiate an investigation. – A root cause analysis is conducted, examining environmental conditions, equipment use, and procedural adherence. – Corrective actions and recommendations for preventing similar incidents in the future are documented. – All investigation findings are entered into the central safety database for future audits. 4. Follow-Up: – The injured employee is provided with appropriate medical attention, and follow-up assessments are scheduled if needed. – Supervisors and HR review the incident to determine if further training or changes in policy are necessary. – All steps, from the initial report to the final corrective action, are documented in a follow-up report. Direct Support Professionals and All Staff: – Must report injuries immediately and assist in the investigation process. • Supervisors: – Ensure that incidents are documented and initiate the investigation process promptly. – Provide guidance on immediate corrective measures. • OHS Representative/Health and Safety Manager: – Conduct thorough investigations and coordinate follow-up actions. – Retain all injury reports and investigation outcomes in the central safety system. • HR and Quality Assurance Teams:
