– A regular inventory check is to be performed weekly using standardized inventory forms. – Any discrepancies or missing items must be immediately reported to the Pharmacy Coordinator. – All medication orders and refills must be submitted through the approved electronic system, with confirmation receipts stored in the inventory log.

Disposal of Medications:
– Expired or unused medications are to be disposed of following state and federal regulations, including the use of secure disposal bins and licensed medical waste disposal services. – Maintain a disposal log that documents the drug name, quantity, date of disposal, and disposal method. – In cases of recall or special disposal instructions, the Pharmacy Coordinator will provide updated guidelines and training.
C. Medication Documentation and Reconciliation
Policy Statement: Accurate and timely documentation is critical for ensuring medication safety and maintaining regulatory compliance. All medication administration, changes, and reconciliation activities must be comprehensively documented to promote transparency and enable prompt identification of any discrepancies.
