Implementation Procedures:
1.
Comprehensive Evaluation:
– Conduct a detailed speech and language evaluation using standardized diagnostic tools. – Develop an individualized speech therapy plan that addresses identified deficits in communication and/or swallowing function. – Engage the client and family in the evaluation process, emphasizing the importance of self-advocacy in communication. 2. Therapy Session Initiation: – Schedule initial speech therapy sessions and provide an orientation describing the session structure and expectations. – Document baseline performance indicators and specific therapy goals (e.g., improved articulation or language fluency). 3. Ongoing Session Delivery: – Implement therapy sessions using a combination of individual exercises, group activities, and technology-assisted interventions. – Maintain detailed session logs capturing session performance, client engagement, and any adaptive strategies used. – Periodically reassess progress using both qualitative feedback and quantitative testing. – Adjust techniques and support interventions as necessary, based on measurable outcomes. 4. Service Conclusion or Modification: – Conduct a final comprehensive evaluation when initial therapy goals are reached. – Document these outcomes and outline recommendations for ongoing maintenance or additional intervention if necessary. – Secure all records in the digital repository with proper versioning for compliance.
