enabling them to remain in their homes. This includes assistance in personal hygiene, meal preparation, medication management, and other everyday tasks.
Implementation Procedures:
1.
Pre-Service Assessment:
– A home-based assessment is conducted to determine the specific support needs of the client. – Develop a detailed personal care plan that captures daily routines, support intensity, and specific interventions required to achieve independence in activities of daily living. – Engage the client and family in an orientation session to clearly establish expectations and services to be provided. 2. Service Initiation: – Upon finalizing the personal care plan, assign a dedicated Personal Care Assistant (PCA) who will work one-on-one with the client. – Schedule daily support visits and incorporate flexibility to adjust timings based on the client’s needs. – Set up an electronic service log to capture start and end times of each visit and record any incidents or client feedback. 3. Ongoing Support Delivery: – The PCA follows the pre-defined routine, recording every action (e.g., help with grooming, assistance with mobility, meal preparation) in the digital log. – Daily supervision is provided by a Care Coordinator who reviews service logs, monitors compliance with time limits, and checks that all personal care tasks are delivered according to plan. – Periodic evaluation meetings (weekly or monthly) assess the effectiveness of the service and identify any modifications needed to maintain optimal care levels.
4.
Service Conclusion or Adjustment:
– When the client reaches a new level of independence or if additional services become indicated, a formal review meeting is convened. – Update the personal care plan accordingly, either reducing the hours of assistance in areas of improvement or increasing support if the client’s condition changes. – Document any changes and secure all previous logs in compliance with regulatory guidelines.
Staff Training Requirements:
• PCAs must receive intensive training in personal care techniques, communication skills, emergency procedures, and data documentation.
