ii. Supported Employment
Purpose: Supported Employment services are intended to assist clients in accessing competitive job opportunities, developing vocational skills, and ensuring long-term job retention, while providing necessary on-the-job support.
Implementation Procedures:
1. Pre-Employment Assessment and Planning:
– Conduct a thorough vocational assessment to ascertain individual skills, interests, and areas of required support. – Develop an individualized employment plan that specifies job coaching, work hours, and measurable employment outcomes. – Coordinate with local employers and vocational training programs to identify suitable job placements. 2. Job Coaching and Placement: – Initiate pre-employment training sessions that include resume writing, interview preparation, and social skills training. – An employment specialist or job coach is assigned to support the client during the job placement process, ensuring that reasonable accommodations are secured. – A trial employment period is established during which job performance, support needs, and integration with the workplace are closely monitored.

Ongoing Employment Support:
– Provide on-site job coaching and periodic evaluation of vocational performance. – Job coaches maintain detailed logs documenting progress, challenges, and any adjustments required in job supports. – Regular supervisory meetings between the job coach, the employment program coordinator, and the client’s care coordinator ensure that long - term employment stability is maintained. – Adjust the individualized employment plan based on periodic performance reviews and employer feedback.
