ii. Supported Living

Purpose: Supported Living services assist clients who prefer a community-based living arrangement while still benefitting from structured support. This model emphasizes independence, personal choice, and flexible service delivery.

Implementation Procedures:

1. Pre-Admission Assessment and Home Evaluation:

– Conduct an in- home assessment to identify the client’s living conditions, daily support needs, and target areas for improvement. – Develop an individualized plan that outlines the level of support, specific services (e.g., medication management, meal preparation), and tools to enhance independent living. – Verify all necessary documentation such as prior service records and in- home assessments. 2. Service Initiation: – Organize an orientation for the client and family, including checklists that explain the nature of Supported Living services. – Assign a Supported Living Coordinator and primary direct support professional. – Initiate a trial period during which support services are provided while closely monitoring the client’s needs and progress. 3. Ongoing Service Delivery: – Schedule regular in-home visits, ensuring that daily service logs are maintained. – Monitor service utilization using electronic scheduling systems that track activity hours against pre-approved limits. – Adjust service intensity dynamically: if progress is rapid, reduce frequency; if additional support is needed, seek pre-authorization for increased service hours. – Maintain open lines of communication with the client and family through periodic reviews and digital check-ins. 4. Service Conclusion or Transition: – When the client demonstrates sustained improvement in independent living, a final evaluation is conducted. – Transition planning initiates potential reductions in support while ensuring that adequate safety nets are maintained. – Prepare a comprehensive summary of service outcomes, document final service logs, and archive all records per regulatory requirements.
