Person-Centered Planning Stages

The person-centered planning process is divided into several key stages:

Pre-Planning and Engagement:

Comprehensive Assessment: – Gather detailed information about the individual’s skills, preferences, support needs, and personal aspirations. – Utilize a range of assessment instruments, including clinical evaluations, cognitive tests, adaptive behavior assessments, and input from family and caregivers. – Document baseline data that will guide not only service planning but also future evaluations of progress. 3. Goal Setting and Prioritization: – Establish clear, meaningful, and measurable objectives anchored in the individual’s personal aspirations. – Incorporate client and family input to determine short- and long-term goals. – Identify both existing strengths and areas requiring support, framing objectives in a positive, reinforcement-based manner. 4.
