Person-Centered Planning Stages

The person-centered planning process is divided into several key stages:

Pre-Planning and Engagement:

– Engage the individual and their support network early in the process. – Share an overview of the ISP process to build trust and foster open communication. – Ensure that comprehensive consent and privacy briefings are completed in alignment with HIPAA and other confidentiality guidelines. 2. Comprehensive Assessment: – Gather detailed information about the individual’s skills, preferences, support needs, and personal aspirations. – Utilize a range of assessment instruments, including clinical evaluations, cognitive tests, adaptive behavior assessments, and input from family and caregivers. – Document baseline data that will guide not only service planning but also future evaluations of progress. 3. Goal Setting and Prioritization: – Establish clear, meaningful, and measurable objectives anchored in the individual’s personal aspirations. – Incorporate client and family input to determine short- and long-term goals. – Identify both existing strengths and areas requiring support, framing objectives in a positive, reinforcement-based manner. 4. Plan Development: – Develop a comprehensive ISP that outlines specific services, supports, interventions, timelines, and responsible team members. – Clearly detail which services (e.g., personal care, counseling, therapy, transportation, residential supports) will be provided, how they will be delivered, and the expected outcomes. – Incorporate mechanisms for regular follow-up and monitoring, ensuring that services remain adaptive and responsive. 5. Review and Adjustment: – Establish periodic review meetings to assess progress, with formal review dates typically every six months or as needed if significant changes occur. – Adjust goals, interventions, and services based on updated assessments, performance feedback, and changes in the individual’s life circumstances.
